MEDICATION AT SCHOOL

 If your child requires medication to be administered at school, we must have a form completed by parent and physician.  This form is located in the purple column ("Prescription Medication at School").  Download this form and take to student's physician for completion, then return to the school nurse. 

Remember, medicine must be in labeled pharamcy bottle and brought to nurse's office by the parent along with the completed forms

Please complete the Medical Information on each student in the enrollment packet.   THIS MUST BE DONE EACH SCHOOL YEAR. 

If completing online, be sure and complete and date, then submit. 

The form may be downloaded (see link in purple column on right) and turned in to the school nurse. 

Varicella (Chicken Pox) Vaccine

Catch-up Campaign

For Adolescents Age 10-18

 

Vaccine is available

FREE OF CHARGE

for those who need their

2nd dose of Varicella*

(including those that have not received their 1st dose)

 

There are no income eligibility requirements to receive the vaccine.

 

Varicella Vaccine is Available at the Following Health Department Clinics:

 

Augusta Clinic, 2100 Ohio, Suite B

Wednesdays – 1:00 – 4:30 p.m.

 

El Dorado Office, 206 N. Griffith

Mondays – 8:30 – 11:30 a.m. & 1:00 – 5:00 p.m.

Last Friday of the Month –

3:00 – 5:00 p.m.

 

*Schools require students to have 1 dose of the Varicella Vaccine.  The 2nd dose is recommended by the Kansas Department of Health & Environment Immunization Program.
